Public vs Private Transportation Network Accessibility and Maternal-Infant Health Outcomes Across the Urban-Rural Boundary in Kalamazoo County, Michigan

The overarching goal of this project is to improve maternal and infant health outcomes in thorough analysis of public and private transportation network accessibility. Disparities in health care between rural and urban areas in the U.S. are widespread, exacerbated by inequalities in accessibility to appropriate care. Population density gradients are not an isolated phenomenon in the United States; 77 percent of US counties have been designated both urban and rural by the US Census Bureau. Kalamazoo County, MI is one such urban-rural county and has exceptionally high rates of gestational diabetes among expectant mothers and high disparities in the quality of birth outcomes among various segments of its population. The authors propose to use spatial statistical modeling to examine the associations among public, such as bus, and private, such as personal vehicle, transportation network accessibility with maternal and infant health outcomes, especially among those on Medicaid, in urban-rural counties. Initial model development will focus on Kalamazoo County, where poor outcomes and disparities are well researched. This project is a preliminary analysis of these associations and could lead to greater understanding of transportation linkages to healthy birth outcomes in regions and populations across the country.
